Where to buy disc conversion

I am considering converting my car to disc after I saw a members Barracuda destroyed by a brake failure. I think that I will need to take my time because I would like to maintain the 5x4 inch bolt pattern. most of the kits have the 5X4.5 inch bolt pattern.
