When checking out a car, open the hood first and check out the engine before starting it. Feel the radiator (inconspicuously) while you are under there to see if the seller "warmed it up for you" before you got there. A sign that it's hard to start cold...

If the radiator is cold, then it's the first start and you can evaluate how it is on a cold start...
