how is the shaft the retainer is on?

any possibility that the actuator arm is hitting the ring the retainer is supposed to keep on the shaft?
 
That is the heavy duty spring so see if you can find the HD retainer to use this time. :)
 
I remember the first time it happen, you replaced it with a stock retainer. I followed your build closely and I went with the cope billet servo and retainer based on your misfortune. You can add that with out pulling the trans. Just Hope you didn’t smoke the clutches or break a band. Sorry to see that happen again.
 
That spring cup appears badly deformed. It looks like it got fired outta there by a cannon. If it is, then you have a hydraulic pressure mismatch.

I bet it happened in reverse.

In manual low, the factory pressure there is around 55psi, but in
reverse there is no pressure regulation, so that servo sees whatever the pump puts out, which is spec'ed at ~250@1600rpm. The hi-pressure relief on the factory VB is set to 275psi .If that piston is 2 inches in diameter, then 275 psi is 864 pounds of force. Ima thinking that spring cup just gave up. Better check your hi-pressure relief system.Or your regulator valve which controls the pressure below the 275 relief.
If the relief is stuck, and the idle is very high, I have no idea how high the pressure could get to be.

Have you been setting that rear band clearance at the "loose" end? The piston would slam down with more momentum every time you put it in reverse. I like to run those as snug as possible to limit piston travel and momentum. Is that a double wrap band?

Jeff you Sir are supposed to be on the coach watching football drinking a beer. I can’t believe the you ripped it out again. You are certainly my transmission role model. What you got planned for it? New valve body?

Well.....while im at it.....!
3000k stall converter, HEAVY DUTY spring retainer, shift kit and the correct 39 tooth speedometer gear!
Oh, and a trans cooler.....

Well that’s exactly what I got 3k PTC 11”, cope billet rear servo, I will admit that the 3k stall soften the shift. I should have maxed out the orfice holes on the TF2 kit. It’s a lot of stall for my wimpy cam and 3.27 gears.

Sorry to see this again Jeff. Make sure when you do the TF2 kit you take all the valves out and clean the VB real good and make sure the valves work free. The HP relief could have been stuck as AJ said.

Were you running a stock converter before? If so the 3k stall should really pep it up
