Doctor Diff offers a kit with the late disc setup redrilled to the 5x4" pattern, as well as all the peripheral parts you'll need for the conversion.
Be aware when you're shopping these parts that the late rotor also needs to have the bearing hub turned down to fit the 5x4 center register, and not just the bolt pattern itself redrilled. The Dr. Diff kit handles that.
