remove the fenders, the hood, the windshield. Next get a spot weld drill and get to work. This isn't a simple task. It is basically part of the unti body.

No, as stated above it is part of the car. To get it blasted remove everything that is bolted, screwed,glued, or somehow stuck to the car. Cut off/out any panel (quarters, corners of windshield etc..) that you know will need to be replaced, so the blaster will get in all the nooks and crannies. See if they offer any more aggressive material (sand) to blast the rust off too.

I am having a similar dilema, I am painting my duster now and didn't know what to do with the cowl. My solution was to pull the heater box and vent out and I pulled in a towel and spread it out to cover the original paint, to not get overspray etc in there. This is fine for the car I am doing as the original paint/primer is in good condition. On my other Duster this is not the case, there is much overspray from previous paintjobs(and colors). I wonder how the high end resto shops deal with this? Somebody must know....
 
I must have missed something. Why would you need to remove the cowl, is it badly rusted through?
 
Have painter spray inside cowl through the slots then wipe down the top with solvent before it dries or sand the top to remove the heavy coat left behind.

Then just paint the rest of the car.

Some overspray will still get in there but if your slick you can tape off the back side through the vent holes.
