Gary Goers has the correct jack instruction sheet for a '64 Valiant. The one StrokerScamp links looks like it might be correct, but the image in the auction is of such poor quality it's difficult to tell what the quality is like. I've also bought good ones from Osborn Reproductions (Goers and Osborn are both listed at the page I've linked in this post)
