Here's a great video and explanation of how a cam is reground. This happens to be a roller but all regrinds are similar.

I just asked him in the comments how they grind taper on a solid or hydraulic flat tappet. Offset the tailstock would be my guess. We'll see if he answers back.

The grinding wheel is dressed with the desired amount of taper.
