How does one go about changing from a small bolt pattern to a big bolt pattern?

front or rear?

rear....cheapest...redrill axles and drums......best ..buy new axles and new backing plates and drums...

Many ways to skin this cat.
I would guess the most common method on the front is to swap suspension parts from a 73 up A body.
The key parts to the swap is the upper control arm and spindle and caliper adaptor bracket. Wear parts are the rotors and calipers.
You could do the same at the rear by using the entire axle assembly from the 73-76 A body.
Their are numerous ways to do this using non factory
parts thru various venders.
I'll let others fill in those many details.

What I did...

Front, A-Body disc brake kit from Stainless Steel Brake Co., the rotors have both bolt patterns.

Rear (8.75 axle), disc brake kit for B-body from Stainless Steel Brake Co., plus 8.75 A-Body BBC axles from Doctor Diff.

i bought a 74 dart swinger for all v-8 swap parts and the front disc setup and the 8.25 rear were a bonus.

I'm going through a bolt pattern swap now. I'm redrilling rear axles and drums and re using those. I swapped my front drums to 73 discs with big bolt hubs.
