Lakewood bellhousing problems

Have a 1972 Duster with a 416 cu.in. It has a Lakewood bellhousing P/N 15320 on it and the clutch fork seems to be too short causing the release bearing to come off the fork when the clutch pedal is depressed. Have confirmed the fork is correct and Lakewoood assures me there it should work with stock fork and pivot, but no luck. Anyone out there had any experience with this or heard of any stories.
 
Is the fork snapped into the pivot tab on the inside of the bellhousing? Sometimes you really have to give it a shove to get it to lock in.
 
And if you are getting the fork completely on the pivot it may still be possible to miss one spring clip if the release bearing twists on you. Don't just assume both clips are on, see that they are on. Mirror on a stick and a light might help.
 
Did you have a different bell on the car previously?

Did the clutch work OK with old bell?
