Oil Filter Adapter

Overtightening the center bolt can do it, especially if the was assembled without the center cork gasket, or, with the thin paper gasket (which I've seen many times when tearing an engine down). The standard steel filter plate was famous for that problem. You tighten the bolt without that gasket, you can warp the plate, or, adapter in this case. Then later when you try to install the correct gasket in the center, it squashes flat, but, the other gasket doesn't. Sounds like that isn't your problem if you have some space between the center boss and adapter hub when the outer ring is up against the block and without any gaskets installed.
