American Racing Hopster is the style but I don't know if they make small bolt or not. Hard to get at stuff on my phone.

E.T. Wheels, American Racing, Cragar, Wheel Vintiques, Centerline, etc. There are plenty out there new and used. Worth some phone calls to manufacturers or Summit.

That's probably "about it" cept for used.
Nobody has mentioned Coys Wheels yet. They used to make 5 on 4".

Probably doing good to find a 15 inch wheel in 5 x 4.5 anymore!!!!
I bought 15 in sbp Cragar SS from Jegs in 6 & 8 widths. They had other widths. Had them on my 68 40 years ago. They were expensive but I love them!

Here is a company that says they have 5 x 4 pattern. They must buy blanks and drill them themselves.

I have steelies on mine, 15x7 on the rear from Wheel Vintiques, ordered through Summit. The fronts are 15x6 from Coker ordered direct. Cragar makes somealso. All SBP. I really like these steelies, they are well made.

Only on some styles can they do a 4" pattern. Call them and check. They manufacture the wheels in CA. A real high quality product.
I think you can see on the wheel charts which ones are small bolt.

The charts are not crystal clear. If you call them you will find out much more than you can on the website. I have dealt with these guys in the past, and they are great.
These are the ET-V wheels with 4" b.c. I had made by ET several years ago. Please note they use some unique lug nuts in order to clear the center hub. (long cylinders, with a hex inside one end, and threads in the other end) :
