I agree with daves69, that's how I've always installed them.

After thinking about this could it be a dry pilot shaft bushing? With clutch not engaged/ pedal depressed tranny input shaft would not be rotating while crank would be if engine is running. T/O bearing off clutch fork very possible as others have suggested.

New or not, based on description of when it occurs, only with clutch depressed, it sounds like it's throwout bearing related.

Not all that uncommon with heavy use and/or stronger pressure plate springs. I've broken several of these over the years but in my cases it was usually the long arm breaking off the tube rather that the tube failing as in your case. Sadly the welds on these assemblies are not that consistent...

Attached is a pic of the oddest looking pressure plate, a 9" slant 6 type, that I ever saw. You'd think in 30+ years and at least 100 clutch changes this would have come up before. Does anyone know what this odd ball is?
